I certify that the information provided in this application by me is correct and understand that falsification of any information on this application or any interview or resume may result in denial of employment or immediate dismissal. I authorize the references listed above to give you any and all information concerning my previous employment and any pertinent information they may have, personal or otherwise, and release all parties of all liability for any damage that may result from furnishing information to you. In return for being considered for employment, I release Daddy O's LLC from any liability which might arise from any investigation of me. I understand that nothing contained in this employment application or in the granting of an interview creates an employment contract between Daddy O's LLC and me for either employment or the provision of any benefit. If an employment relationship is established, I understand that I have the right to terminate my employment at any time and that Daddy O's LLC retains the same right.
